What's Inside

The guide is organized into four sections so you can jump straight to what you need.

Section 1 — Seed Forms

Pre-built forms loaded in every Ritten environment. Use them as-is or customize them in the Form Builder. Categories include:

  • Clinical notes & documentation

  • Assessments & screeners

  • Treatment planning

  • Admission / discharge / transfer

  • Consents, legal & financial

  • Medical & nursing

  • Level-of-care criteria

  • Other (behavioral contracts, referrals)

Section 2 — Standard Sections

Reusable, modular building blocks you can mix and match to create custom forms — demographics, psychosocial history, vitals, screeners, and more. Some sections auto-sync data with the client chart (demographics, medications, diagnoses). The full catalog is listed alphabetically in the guide.

Section 3 — Form Builder Walkthrough

Step-by-step instructions for creating forms, configuring signatures, enabling portal delivery, setting due dates, and more. Covers question types, calculated scoring, the Answer Library, required fields, and how to load previous answers into a new form.

Section 4 — Form Instructions

How to embed documentation guidance directly into your forms so staff see expectations while they chart. This section also explains how Form Instructions power Ritten Intelligence — including the AI Form Reviewer, AI Scribe, Note Summarization, and Improve Text.

How to Use This Guide

  • Download the PDF above and keep it bookmarked for your team.

  • Browse seed forms to see what's already loaded in your environment. Each form has a direct download link to load it into Ritten.

  • Explore standard sections to find reusable building blocks for custom forms — the full catalog is listed alphabetically.

  • Follow the Form Builder walkthrough when you're ready to create or customize a form. It covers question types, scoring, signatures, portal settings, and due dates.

  • Check back monthly for the latest version. New forms and sections are added regularly and this guide is updated to reflect them.

Pro tip: Start from a seed form and modify it rather than building from scratch. Use smaller, focused sections so data carries forward cleanly between forms.

Common Questions

What are seed forms?

Seed forms are pre-built forms that come loaded in every Ritten environment. They cover the most common documentation needs — intake, assessments, progress notes, treatment plans, consents, and discharge workflows. You can use them as-is or open any one in the Form Builder to customize it.

What are standard sections?

Standard sections are reusable, modular form sections — think of them as building blocks. You can add them to a seed form for a quick customization, or combine them from scratch to build a fully custom form. Some sections auto-sync data with the client chart (demographics, medications, diagnoses).

How often is this guide updated?

The forms library booklet is updated monthly to include any new seed forms and standard sections added by the Ritten engineering team. Download the latest version from this page to make sure you have the most current catalog.

Where do I build or edit forms in Ritten?

All form editing happens under Configurations in Ritten — there is no other place in the platform to find or edit forms. The Form Builder walkthrough in the guide covers every step.
